Whether you live here locally, or are visiting the Outer Banks, all new clients are required to fill out a registration form. For your convenience and to shorten your check in time we have registration forms available online. Follow the directions that pertain to you below ...

Never Been Here Before?
We will need information to set up your pet's new record. Click our Registration Form link to open, print, and fill it out.

Remember to bring a copy of your pet's current vaccinations. We can copy original documents at the hospital, if needed.
Please go to the boarding section to view the required vaccinations for boarding.

Boarding With Us?
Each time you board your pet, we will need your most current information about your pet. Click the Boarding Form link to open, print, and fill out before you arrive to save some time.

Please be sure to include a local, or cell phone number where we can reach you, if needed. We also ask that you provide any information regarding diet, special requirements or any current medications or pre-existing conditions, so that we may better meet your pets needs.
